HOW TO REFER YOUR PATIENTS A hardcopy of our referral form can be downloaded (see opposite). Alternatively please complete the online form, this will allow us to respond to your request with more efficiency. Once a patient has been referred we will contact the patient directly to arrange a consultation. Following the consultation a full written report will be sent to you and the patient. A written report outlining the progress of the patient will be sent to the referring practitioner and at the end of treatment we will forward any before and after photographs if required. During the treatment patients are always reminded to continue to see their own general dental practitioner for regular routine examinations and any necessary treatment. CODE OF PRACTICE • Referred patients will not be accepted for any other treatment other than that which the referring dentist has requested. • A full written report will be sent the referring dentist and a copy to the patient. • No treatment will be carried out without an estimate of fees given to the patient. • If you have any problems/queries we will endeavour to address them as soon as possible.       I can offer a confident and expert approach towards the periodontal and implant care of your patients without compromising your relationship with the patient.
